Holmlands Medical Centre is fully committed to achieve the best health for our patients including weight management.
NHS England are going to pick up patients from our list who qualify for that criteria, hence we are not able to prescribe Mounjaro or any weight reducing injections directly.

02/07/2025 – Living Well service
The Living Well service is built on the principles of listening to communities and meeting their needs by bringing the right services to them. The service, delivered by Cheshire and Wirral Partnership NHS Foundation Trust, operates on a fully drop-in basis. It offers all routine NHS immunisations, health checks and mental health support. 01/07/2025 – Self-referral system launched for health coaches
Patients can now self-refer to see one of our health coaches for help with their weight, cholesterol, and/or diabetes. For more information, click here to visit the health coach service page on our PCN website.

Sometimes GPs and ANPs have to deal with very sick patients and a referral has to made to hospital which does cause delays. We have a duty doctor dealing with urgent requests but sometimes patients on pre-bookable appointments can also have life or limb-threatening situations.
If you can’t wait for any reason, please rebook. The surgery can’t update and text all patients booked on the day at every hour about the delays which are not in the health professionals’ control.

It is now a new requirement for GP surgeries to share records of deceased patients with the medical examiner before issuing a death certificate.
